The movie was actually titled something else but changed to eyes wide shut before release.

Because Bill may be rich outside NYC, but in the bubble that is NYC, he's basically strictly middle class in the eyes of the 1% elite.

That's one of the major themes of the film, in terms of Bill getting a gut check towards his actual place in the social pecking order. He's a successful doctor yes. But he works almost exclusively as a doctor for the rich and powerful, the 1%ers like Zeigel who make more money in a year than Bill would in a lifetime.

Further, he only survives because Zeigel (his patron, who gives him spillover perks like going to his fancy Christmas parties in exchange for cleaning up ODing hookers in his bathroom and not telling anyone about it) considers him a useful idiot at best. And at worse, someone who knows that he is utterly expendable and that his cushy cosmopolitan lifestyle entirely depends on not pissing off the rich people he does housecalls for.

The story Zeigel spins for Bill, alone shows a level of tacit "I'll tell him a bunch of lies to shut up his curiousity and guilt" knowledge of how Bill's kind work. They'll continue to play the part for the elites, so long as they can be presented with enough plausible deniability to live with. Zeigel can easily find a new doctor, but on some level Zeigel would rather just keep Bill and not have to break in a new doctor.

Bill isn't a millionaire, not by the standard of NYC and it's gouging.

He's basically pulling at best, barely under a million and he doesn't really have anything to show for it. His house basically is crampt as hell and he can't even afford to keep afloat his wife's failed art gallary, let alone warehouse her unsold inventory (which is laying around the place). They can't even afford a nanny, just teenage girls who babysit for them while the wife has to be a stay at home mom.

Granted, he's doing better than most common rabble in NYC, but he's strictly middle classer/upper middle class, having hit the glass ceiling.
