In the books, IIRC, the women and children had already mostly been sent away from Minas Tirith before the siege. Pretty much the only women left were in the Houses of Healing, I think.
I'm not sure there were any women at Helm's Deep. Eowyn and the women had all been sent to Dunharrow when the host rode from Edoras. Might have been a few left in the Westfold that took refuge in the Deep, but I don't recall any mention of them. There were no elves there (aside from Legolas, obviously) either: that was Jackson's own invention.
But there WERE other ways out of the caves. Secret passages existed which were explicitly discussed, but Theoden said he didn't trust them because he expected Saruman had had time to discover them.

Because women then and now couldn't effectively wield a 5 foot long sword and wear 60 lbs of chain-mail and it would be a fucking waste of valuable resources to equip them all. They literally wouldn't have enough weaponry to even let them use it, well crafted swords/armour wasn't common in the mid ages

This. Most effective use of women is in a supportive role. Like in ww2 when the majority of munitions factory workers were women. Small hands plus thousands of years of experience in doing menail work in the kitchen meant women were ideal for assembling bullets, tank shells and bombs.
