It's made clear that people like Patrick are interchangeable, their desire to fit in and social obedience making them into copies of one another
"I want to fit in", there being no "Inside" or "Real me" only appearance and comparable lifestyles, Bateman having an identical hairstyle to Marcus Halberstramm.
The point is that he's in a dehumanizing environment that takes away individuality and forces a kind of competitive insanity that also creates the perfect psychological breeding ground for contempt and the perfect hunting ground to take that out.
It's easy to kill someone when he's indistinguishable from everybody else.
Or it's all in his head, because it could be, because he's so isolated and dehumanized he doesn't even think like a person anymore.
