When I first watch Goodfellas, I didn't know that the mafia has hierarchy or there are "made" guys. I always thought Jimmy was a high-ranking member in the mafia and not in associate as I found out later.

I noticed this too. The first time he shoots stacks, Carbone is standing in the doorway watching but the second shot he isnt. Not really a inconsistency but just Scorcese and his unique directing.
carbone is just one thing. but in real times he shoots him 4 times (1 + 3 off-screen) and in the slomo replay you see 5 shots.
