Standard of living is usually affected by your productivity. If you can make more stuff to either consume or trade for more stuff your standard of living will be higher. Of course there are some special cases like being a tax haven or the issuer of the reserve currency of the world in which case you can essentially trade "nothing" for stuff.

That is a difficult question to answer. Easy way out is to say that we had a strong manufacturing economy after world war 2, while the rest of the modern world was rebuilding. Asian nations weren't even close to modern, and there seemed to be less money influencing politics. Another major factor was women joining the workforce. Before that time period, few women worked. Men provided for them, and they formed a single household.
But, of course, this changed not too long ago.
Now, there are more people, less households, and the same amount of land. So, of course, real estate increased in value. So now we have lots of single people running around, spending 60% of their monthly income after tax on housing, when they could be in a mutual relationship and actually afford to buy a house. But this is not realistic when selfishness and greed are seen as good things in modern society.
So less competition and less easily realized greed meant better living standards in a bygone era. Right now, we are coasting downhill. We are still pretty high up there in standard of living, but the reasons as to why we were there have fallen to the wayside.

The issue is that what ends up happening is wealth concentration at the top because all jobs that can be outsourced are and the only ones left are either middle class or upper class.
