There’s several examples, in both series, where he gets them to change their ways and a month after he is gone they change back and the restaurant fails.

The British version does look like the real deal.
Producers probably tell them not no clean anything and come to check before shooting. If its not dirty enough to create dramatic TV, they are out.

What are the most notable failures from the show? I haven't watched that much of it, but the little I've seen, it did seem like the restaurant owners usually managed to learn something from him.

I remember an episode with some guy and his Swedish BF who had a B&B and the guy filled it with the most hideous and expensive furniture and art pieces he could find and were about a mil in debt for some inexplicable reason. Ramsay fixed a lot of it and told him to stop trying to make a museum of ugly shit, but as soon as he left the guy bought more stupid shit from their new revenue and they ended up selling everything.

I remember one about a pub in the UK, where they did a real gravy campaign. One of their issues was the owner doing everything because he couldn’t let anyone else do anything, they had a guy to cook the food, but the owner insisted on doing it, only him, and he wasn’t that good at it.
On the return to restaurant he tracks down the chef and finds out the owner went back to his old ways soon after Gordon left.

Another one was this guy who used to have a Michelin Star and had a different restaurant now but insisted on cooking like it was the 90s (small portions and decorated) and bought really expensive ingredients and was in debt because people didn’t go to his expensive restaurant and when Gordon left I believe he went back to his imported hams and increased his prices.
There was a guy that had a dish that Gordon is credited for inventing on his menu, obviously Gordon orders it and it's shit.
That's the guy who kept all the old books he was mentioned in right? I touhgt he turned his shit around after his fancy shit got turned into a pub with a slightly bugger menu.
I have no actual idea why we get these over the top dramatic television shows like kitchen nightmares, its obvious that its heavily cut compared the UK version.
I genuinely believe that they really just cherry pick when it comes to the american version, I have never seen a soul that delusional and I live in Texas which is apparently composed of Ranches, COWBOYS AND GUN FOLKS according to the media.
