According to the Wachowskis sisters, Neo interrupts Cypher writing a script (some kind of automated operator) that will enable him to enter and leave the Matrix without a human operator present.

As to how he got the jack into his head, we're left to assume that he simply reached behind himself.

That's one of the reasons the trilogy fails, because it's conclusion really didn't change anything much, we're back at square one, humanity is still fucked acting as Duracell batteries and the machines are still playing their silly deceptive games.

I don't think so. Neo using his powers outside the Matrix demonstrated that "the one" that the machines used to exert control was not the same "one" that Neo was. Neo defied everything known. He was truly The One, in a world where The One was not supposed to exist.

Coming back to life means there is no spoon, you have not been just killed by an agent, the agent is just data, it´s not real and as such it can not hurt you. Coming back to life is the ultimate proof that all of it is in your mind because it´s impossible IRL. If you can do it it means you are in a simulation. Most rebels know this but the simulation is just too realistic for them to be able to truly feel any diference.

Persephone revealed another interesting aspect of the machines when she asked to kiss Neo in Reloaded, she thrived for a fresh sensation. The machine may have won, but they have to endure an eternal routine of Matrix's maintenance that is bound to bore them to infinity. You can only polish a code to complete perfection, but then what is left to do? Smith was very aware of this phenomenon and did not wanted to go back to the source, that was in fact his fundamental drive for his every action in reloaded and revolutions.
