I loved that Animorphs arc where they spend three books trying to infiltrate a meeting of world leaders because they know their enemies are going to, but shit keeps going wrong over and over. Finally, emotionally exhausted from everything that's been going on during the arc, they go for the simplest solution: Turn into rhinos and smash random shit until the meeting is called off.

Hey, I know of a certain cartoon with girl MCs in which the bitchy girl gets much of the slapstick...

One time I was at a monster truck rally and someone was doing a stunt in a drag car (they were gonna jump a bunch of junked cars in one of those really long, thin drag cars) and something happened with the steering and they veered off the ramp and slammed into one of the cars they were going to jump over (it was stacked wide).
Everyone was silently because it was clear what was going to happen as something was off when they started moving, so you just heard the driver scream "MY LEG!" a moment after the crash happened and before the audience started shouting.
It was hilarious.

How did they fix that?

When a superhero gets an upgrade of some kind but then finds themselves in the one and only situation where said upgrade is a liability so they never fucking use it again

I think you're thinking of the modern version of The Time Machine.
He can't save his fiance from dying because her death is what directly led to the invention of the time machine to begin with.
