I haven't heard any solid theory on how or why the polls were so far off. As much as I hated Bush winning in 2000 and 2004, it wasn't a total shock because the polls indicated close races. Hillary had a 3 to 6 point national lead. It looks like she'll still have a national lead but around 2%. She was winning in EVERY poll in Wisconsin and Michigan - neither team from either campaign spent much time in either state. Pennsylvania was contested but she had a consistent 3 to 6 point lead there. All those states she loses by 1 point.
Oh, she was also winning in Florida (lost by 1 point) and North Carolina (lost by 4 points).
What will be most haunting in hindsight is that IF we had polls in the ballpark of what actually happened nationally and at those states, Hillary's team could have withdrawn completely from Iowa, Ohio, Arizona, and North Carolina and focused on Wisconsin, Michigan, Florida, and Pennsylvania.
If she had put forth the same effort in those states, I think she wins. They were all decided by less than a point. GOTV and ground game would have prevailed.
