Again, you don't vote directly for the chancellor but for the party in the federal elections. So CDU voters who don't like her will still most likely vote CDU. Also, yes she had the worst approval ratings, but she recovered after she made some concessions and admitted that her policies weren't the best yada yada. Also while her approval might have sunken among conservative voters, the voters from the socialdemocrats, greens love her for her lefty policies. Last but not least the approval ratings of the other parties candidates are even worse.
Further note: the CDU doesn't have an alternative to Merkel, because over the time every serious contender or up-and-coming colleagues were politically eliminated.
