EXTREME cost cutting. Madhouse counts very few true employees for a company its size, because few animators are tenured. Most are either individual contractors or Madhouse hires another studio (originally Korea, but now Philippines) do to it even more cheaply.
Having to not pay animators but still demand a lot from them has lead to acceptable quality final products on inexpensive budgets. Look at how much free animation was donated in One Punch Man.
Madhouse is basically on its second life anyway. It nearly died in the early 2000's, unable to recoup losses from anime in the decade prior, until it was bought out. Then Madhouse kept a low profile with niche titles and some cutscene work until it got Hunter x Hunter.
Even now, Madhouse's primary role is as a middleman. American companies farm out animation work to Madhouse who then delegates it to DR Movie. Madhouse actually did nothing.
Studios are freelancers. Brains Base did the first season of Durarara, and then the director started Studio Shuka, where he did the rest of DRRRR!! with the exact same Brains Base staff.

The studio that outsources regularly to Philippines is Toei Animation.

Nonsense. X-Men, Blade, and the two Marvel OVAs (Iron Man, and Black Widow & Punisher) were for the most part animated at Madhouse.

I remember having the storyboards, photography, and the color desing done by mostly by in house people. Also for animators I think that Sawada was on it. Also people like Minowa or Kanemori but they aren,t as flashy to be noticed.
