Yes there were, they were literal quests that updated. They always start out with a corpse and once you read the note found on the corpse the quest activates

I spent 140 hours with the game on Death March, cleared out all of Velen and half of Skellige. The combat was exactly as stated in that post, plus oils and bombs, which were tedious at best.

There is literally no need. Only reason why there are monsters everywhere is for game play reasons. According to lore monsters are not common and are getting pushed further and further away from civilization. Geralt mentions this a lot in the books and it also even says it in the loading screen for the third game.

Just the expansions. The other DLC is simply cut content that they released on a weekly basis for PR.

Just buy the expansion pass, it's cheaper than getting the two expansions separately.

They're all self contained but heart of stone fits best in the middle of the game and Blood and wine fits best in the end game after the epilogue.
