Old Iron King was a "whack the giant's hands" fight, typically used by sprite-based games when they ran up against their own limitations hard. It's a literal case of "whack its toe until it dies", much like the Ancient Dragon later in the game (though with the dragon there's at least more of a "chop it up like a tree" excuse there).
Also, the Smelter Demon was said to have killed the Old Iron King in one hit. While the boss is a powered-up version of the king, it still feels underwhelming to fight him and not, say, an ultra Smelter Demon or some sentient volcano.
