it's mostly that only one person in the chat makes it super awkward.

It feels a lot more intimate, especially if they don't say anything and expect me to acknowledge it first.

If they join in and just immediately start asking questions or something it's a lot less awkward for me because it just opens up a conversation.

Maybe they don't want to be acknowledged at all and would rather just watch you play?

As a streamer the best thing to do is never directly call out a lurker. Maybe when you're a big streamer with like 500+ lurkers you can do something like "Love to the lurkers out there too." every once in awhile to show them some love but otherwise people just wanna go into a channel and chill.

Do both. I treat streams like television, just find a chill stream or maybe someone blind playing a game I've enjoyed.
