Post rig and desired performance. If it's your CPU that's going to be rough, but a GPU replacement requires no other parts, except maybe a monitor if you want to up your res or a fan if your case is a crapbox.

Desired performance would be being able to play the majority of games on high at 1080p fullscreen with around 30fps or more (for reference, I cannot play the Witcher 2 unless everything is at the lowest possible setting, and even then the framerate is around 15fps, and REmake only goes to around 25fps in some parts when everything is at it's lowest at 1080p). I've wanted to start replacing some of these parts where possible for some time, but the problem is what would suit my needs and if there are some things here that I would have no need replacing.

Do you mean specific models of i5 or an i5 computer I recommend? Any computer with an i5 5xxx or higher should be fine. RAM, HDD, GPU all don't matter at this point, just a computer with a strong processor and RAM 1600MHZ capability, 16GB max or more. Only get a full-sized desktop, no small shit. Lenovo is gold, HP and Dell are silver, everything else is shit.
Once you have a good desktop with a strong CPU, you can upgrade as you go, but with priority.
1. GPU (~1050 GeForce) 2. 8-16GB RAM (1600MHZ DDR3) 3. SSD (500GB Samsung) 4. PSU (Whatever you need to run everything, probably 600W, only get 80+ certified crap)

You probably want to start fresh like the other user said, but you can fix this if your PSU is powerful enough. It's only ~40W more but if you're riding the line already that's game over man. First, new CPU and RAM >AMD FX-8350, ~$120. >Any 4 GB 1600MHz DDR3 RAM stick, ~$40. and then any GPU of your choice, cheapest I am comfortable recommending is a 1050Ti , around ~$160 as well. The problem with this is A) if your PSU is a crapbox it just won't work and B) this isn't a long term solution and your money would be better spent (frames per dollar) on newer stuff, but with that comes a higher total price. Used market for upgrades is a terrible idea on stuff this old just because it has probably already been run to death.
