If you have 5 difficulty levels at the start of the game, then I'd probably start hard and then put it up one if it's too easy.
Most games generally don't have 5 at first though, the last difficulties are usually an unlock.

no, its just that difficulty settings should add challenges and objectives rather than making the enemies easier or harder

rpgs I put to normal, rts I put on easy until I learn the game, shooters usually hard, games like ninja gaiden and bayonetta hardest
Normal. I just assume that's the developers intended difficulty unless there is indication otherwise, e.g. Alien Isolation says hard is the recommended difficulty. If I'm replaying the game and found the first play through too easy then I'll bump up the difficulty.

I've actually played that game. The difficulty is almost irrelevant because you can one shot kill everyone from cover once you get the curve mechanic down.
Whatever I find the most enjoyable, which in this case would be Old School.
Always hard or very hard unless it's a strategy game or sim.
Always the highest to impress my Sup Forums friends.
