It doesn't matter... even when I'm rolling a good character, I kill Roy and his pack of savages every time and gladly take the karma hit. Almost everyone agrees that it's the "good" thing to do, and the karma system is wrong on this part.
Murdering in cold blood is not something a good person does, no matter what the murdered person did.
They are not hostile to you, and pose no threat to you. You decide to murder them because they killed other people.

In the lawless wasteland, there's no such thing as "murder." We're talking wasteland justice here where you don't have the luxury of calling the cops to arrest people like this so that they can stand trial for their actions. If you let a violent group of people who are okay with slaughtering packs of innocents go free, you runs the risk of having them grow in power and influence and become a tyrannical ruler of the area. The greater good would be, in that case, to put a stop to them.

Most characters end up far richer than the folks at Tenpenny Tower, so they're not exactly bootlickers. Further, just because the ghouls are poor and powerless doesn't automatically make them "proletariat." The the ghouls don't work for or provide any services to the Tenpenny folks. If anything, the Tenpenny residents want the ghouls to go away and not associate with them at all. Stop using terms you don't understand in an attempt to elevate your argument

Karma was perfectly conceived in Ultima 4. Not as "being good", at least not 7 of the 8 of the virtues, but defining your adherence to contrasting and overlapping philosophies. Let bandits flee from battle? You're rated for being compassionate but unjust.
I don't bring this up to just gush about Origin. But because I am floored why games like Bethesda's can keep track of every single interaction and in-game action you perform but don't actually incorporate that. People realistically, could hate how you conduct yourself; even if you're on their side. Too happy to kill people? Buy off too many enemies? Habitually convince people to betray their allies? Demand more money every time you complete a mission? That shit should be foundational in reputation.
Instead if one goes through the game picking 0 locks and stealing 0 items but have 100 skill in lockpicking: You get guards mumbling veiled threats about your criminal activities.
