look at the PCIe power connectors, they're not plugged in.
Its possible that the card didnt have robust enough current regulating circuitry on the PCIe bus power in and tried to draw 10+ amps (at the pcie 12v mobo power) through the one or two MOSFETs that normally handle the 75w max from the mobo.
Just a guess though.
Card shouldn't even work if the extra power connectors aren't connected. My GTX 980 refuses to do anything if they aren't connected properly. Even if one 8 pin is connected and the other has higher resistance than usual it refuses to boot. Never mind if it's missing completely.
Depends on the design of the card. Well designed ones will do what yours did, but poorly designed ones will try to work with available resources, and fuck their own mouths in the process.

Nope, PCI-E connector wouldn't be damaged. The most likely effect of a card overdrawing from PCI-E slot is the 20/24 pin motherboard connector being damaged. That is the weakest link.

This is an issue that is only exists in EVGA cards, where the VRM is not cooled well enough. They are addressing the issue by giving customers free thermal pads and bringing out a BIOS update that changes the fan profile.
