Those that are at the bottom reap the benefits more so than those with full or partial coverage. Both social programs implicitly state that they will not pay until the primary insurance does (and maybe not at all after that).

The programs are aimed at the old and poor. Most 'normal' households will be covered by the primary bread winners private insurance (including spouses and children). The social programs are designed for people that do not have this.

I had private healthcare, moved jobs and had no healthcare. I paid the 500 dollar penalty. Had I had a medical issue during that time I would have paid out of pocket.
