I have KDE but unfortunately some common applications like Firefox still force the GTK file chooser.

There's a patch for Firefox. If you're on OpenSUSE it might be default (they maintain the patch). On Arch you can install it from the AUR.

I won't, I never needed it and probably wont ever need it. I'm fine with the small thumbnails and the bigger preview.
Rather than having your toolkit define the file picker, it should invoke an external file picker over IPC (D-Bus)
That way everyone can create a file picker or install a different one.

if you favorite filemanager does not support thumbnail mode you could get an image viewer that does; or just use a backup filemanager for when needing to view thumbnails.
Nomacs image viewer has a pretty good thumbnail mode if you press shift + t
Dolphin and konqueror filemanagers seem to support bigger image thumbnails than most filemanagers do.
