It never got released, 3dfx went bankrupt and got bought by Nvidia before they could get it out the door. But there were a bunch of dev boards made, and that's one of em. V5 5500s did get released, I still have mine somewhere. They look like that only with two chips instead of four (and slightly shorter as a result)
