Just the illiterate. I recently fell for the Gentoo meme from botnet10 being a 100% Winfag. Of course problems arose, make sure to stick to the handbook during the installation and PAY ATTENTION TO WHAT YOU'RE READING, there's always a reason something is written there. After the install, Gentoo (and to a certain extent Arch) Wiki is your friend. If push comes to shove, there are forums and IRC channel to get personal help.
Make sure to go in determined, focused, willing to learn and solve problems.

What does Gentoo offer that makes it worth the effort? What sets it apart?

Portage, Gentoo's package manager, singlehandedly. It offers tremendous amounts of choice thanks to having your machine compile all the packages from source and allowing you to configure what features you want via USE flags (keep in mind that this is also a double-edged sword, Gentoo is not ideal for low-powered machines, unless you set up a more powerful one to compile the binaries for you). You also get the option of compiling your own kernel (not as difficult as it sounds, essentially consists of ticking or unticking a whole bunch of boxes to select or unselect functions and device drivers) to further cut down on the unnecessary and/or unwanted features.
Couple that with the fact that Gentoo, by default, uses OpenRC instead of systemd (you of course still have the choice to use systemd if you wish).
Gentoo is all about user choice. This can be useful and fun but is also time-consuming. You can of course leave on all the default USE flags with all packages and use genkernel to compile your kernel for you, but then why use Gentoo?
