So, I have an idea for a porn app with my own twist to it and already have a developer ready to start working on it. The only issue I'm trying to figure out is how to release it. Obviously I can't release on the app store. I've seen other illegal content type apps being downloaded from Safari browser and you allowing it permissions in settings and it works then.
Anyone have information on how I can do this? And would apple block the certificate if they found out it is a porn app even though it isn't released through the app store?

Serious answer: You need to make a mobile-friendly website, not an app. Yes, pornhub has an Android app. I installed it and it works well. They're big and "trustworthy" enough that they can get away with asking me to install an unvetted APK. You can't. Make a website instead.

Not quite what I mean. I know the technical side of it; how to install APKs and which security setting I need to use. What I'm saying is: Pornhub was able (and only just) to convince me to trust them enough to install it. If OP's random site pops up an APK download, I'd tell it to fuck right off and never come back.
