The book hasn't explained it yet. Shit, it hasn't even confirmed it's The Mountain, just that it could be him since it matches the size. Nobody knows what's under the helmet, which in the book is completely closed. One of Bran's visions implies there's no head at all beneath the helmet.

It's still a ton of power behind a very tiny point, even really thick plate mail should not be able to prevent a weapon like that from piercing a hole with a direct hit right in the center.

Although it would definitely stop the bulk of the weapon when the nail goes up to the hilt, and it's big enough that the point would only barely reach the skin through all that space and padding.
