At far as books go there's a chronology, as for the show the characters keep talking that 'years passed', you don't usually say that when 2 years have passed. Plus they obviously need it to justify the fact that Stark kids grew into ugly teenagers

He's a zombie so the chance that he is injured is small. Besides that it's rare to only wear a breastplate, there could have been multiple layers of gambeson/leather/mail under there meaning that the points that tore his armor didn't reach his body at all.

Plate was generally between 1.5 and 3mm thick, depending on whether or not it was a surface that was expected to be struck. Since this was his vitals, we'll say it was 3mm. That's a tenth of an inch.
