Was the psychiatrist not real too? Or are they going to pull a you can't trust anything since the main character is unreliable, even the scenes he isn't in sort of things?

I do agree on the final sequence bit, it just looked rather out of place in regards to the rest of the episode's direction, cinematography, direction, et al. Though it did seem like a continuous take so that was kinda neat.
That said we've got to reserve judgement until a few episodes in.

It is supposed to take place in the universe but be a stand alone story with no real crossover with the exception of mentioning/referencing Xavier being his father. There are also theories that they might use him as a reason to reboot the X-Men universe because he has that ability.
