Was the psychiatrist not real too? Or are they going to pull a you can't trust anything since the main character is unreliable, even the scenes he isn't in sort of things?

I do agree on the final sequence bit, it just looked rather out of place in regards to the rest of the episode's direction, cinematography, direction, et al. Though it did seem like a continuous take so that was kinda neat.
