BDH has said that the scene they were filming at the time was a really tense scene where she (her character in the movie was pregnant by the way) puts a gun to her head. She said that they were all making themselves tense and angry on purpose to enhance the scene and apparently there was a hurricane coming in within the next few days so they had a limited time to get their shooting done. Basically they were under a lot of pressure (especially they, the actors) and Bale cracked because that guy's little mistake was the last straw.
