What happened to the Polish majority is Belarus and Ukraina?
The same thing that happened to German majority in Silesia and Prussia. They were removed from these territories. You can find their descendants in Silesia and Pomerania.

A lot of them went to live in the "recovered territories". There's still a lot of Poles in Belarus in Grodno Region, but they're minority now. Though there's one district on the Belarusian-Lithuanian border populated in 80% by Poles (Voranava District)
>i know a lot of people whose grandparents were forced to polonize their german last name
These people are Silesian Poles who were forced to germanize their surnames back in the 19th century by the Prussians, after WW2 they were forced back to Polish.
Real Germans got removed from Poland.
USSR moved lots of people around after the war.
