People can and will be empathetic towards their family and closest friends, also to other less close friends as long as their immediate needs are met and they don't need to compete with said friends. But in no circumstance can a person be empathetic towards a whole nation.
And yes, people can and have cared about their ill and disabled and old and their young far before the appearance of the welfare state and socialism. It's called family, a small union of people who care about one another and put the greatest importance on the well being of family members, even over their own selfish personal gain. It was to abolish the need for families by creating public alternatives to the benefits the family currently provides for small groups of people. It's not something that requires anybody to care any more or less about other people than they do now. The point is to change it so the value that family currently provides is not reliant on each person in the world having to create a family in order to be able to experience that value.
